Key Responsibilities
  • Develop and implement comprehensive care plans that address the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure effective communication and coordination of care
  • Monitor patient progress and make adjustments to care plans as needed
  • Advocate for patients and families to ensure their needs are met and their voices are heard
  • Maintain accurate and up-to-date patient records and documentation
Requirements
  • Registered Nurse credentialed from the Michigan Board of Nursing required
  • BLS Provider certification preferred
  • Case Manager certification from the Commission for Case Manager Certification (CCMC) or American Case Management Association (ACMA) preferred
  • Diploma from an accredited school/college of nursing or required professional licensure at time of hire
