We are seeking an experienced Program Specialist to join our team at Active Day. As an ODP Program Specialist, you will work closely with our members to understand their goals and develop individualized plans to support their success.
The ideal candidate will have experience working directly with adults with intellectual disabilities and possess excellent communication and organizational skills.
ResponsibilitiesThe successful candidate will be responsible for:
- Developing and implementing individualized service plans (ISPs) in partnership with Center Directors;
- Coordinating and completing member assessments and goal development;
- Assessing member progress and making recommendations for revisions to ISPs as needed;
- Acting as the lead on ISP development, annual update, and revision; and
- Implementing provider services in partnership with Center Directors.
To be considered for this role, applicants must have:
- An associate's degree or 60 credit hours from an accredited college or university and 4 years of experience working directly with individuals with disabilities, or;
- A bachelor's degree from an accredited college or university and 2 years of experience working directly with individuals with disabilities, or;
- A master's degree or above from an accredited college or university and 1 year of experience working directly with individuals with disabilities.
Prior experience working with adults with intellectual disabilities is preferred.
