Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Three Rivers Home Health Services.

About the Role:

The LPN will be responsible for providing high-quality patient care and performing technical skilled care in the patient's home. This includes:

  • Providing services within the scope of practice as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Completing progress notes for each visit and submitting them to the agency daily.
  • Observing, recording, and reporting to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition.
Requirements:

To be successful in this role, you will need:

  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification.
