LPN - Heart Failure Stepdown Specialist

We are seeking a skilled and compassionate LPN to join our Heart Failure Stepdown team at the Cleveland Clinic. As a key member of our patient care team, you will provide direct and indirect care to patients with varying degrees of acuity, working closely with our RNs and other healthcare professionals to ensure the highest level of patient care.
Responsibilities- Provide direct care to patients, including vital sign monitoring, medication administration, and wound care.
- Assist with patient education and discharge planning, ensuring patients and families are well-informed and prepared for post-acute care.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans, prioritizing patient needs and goals.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to process changes that enhance patient care and safety.
- Graduate of an accredited LPN program.
- Current Florida LPN license.
- Basic Life Support (BLS) certification through the American Heart Association.
- One year of experience in an acute care setting, preferably in cardiology or critical care.
- Knowledge of infection control practices and quality improvement principles.
