Richmond Community Services, a leading non-profit provider of support services for individuals with developmental, intellectual, and behavioral challenges, is seeking a skilled Physical Therapist to join our team.
Job SummaryWe are looking for a highly motivated and experienced Physical Therapist to provide support in our ICF and group home locations. The successful candidate will work under the direction of the Coordinator of Rehabilitative Services to plan and implement prescribed physical therapy treatment programs for consumers.
Key Responsibilities- Conduct annual screens, assessments, and daily documentation to maintain and/or restore function secondary to acute or chronic disability.
- Develop and implement treatment plans based on individual needs and level of functioning in areas such as sensorimotor, gross and fine motor skills, musculoskeletal status, CNS status, ambulation, and adaptive equipment needs.
- Collaborate with physicians and other healthcare practitioners to obtain additional consumer information and integrate input from individuals, families, and direct support staff in treatment assessment and recommendations.
- Administer treatment involving modalities such as hydrotherapy, hot packs, cold packs, ultrasound, and electrical stimulation, and instruct individuals in various exercise programs.
- Write specific, objective, and measurable documentation detailing individual needs, potential to benefit from treatment, and progress achieved.
- Consult with supervisor/manager to implement recommendations and provide training to individuals' families and/or direct support staff in specified follow-through programs.
- Communicate effectively with staff regarding discharge planning from the rehabilitative program and participate in annual and semiannual conferences for individuals receiving physical therapy services.
- Supervise PTA (clinical supervision) and therapists on limited permits, including providing co-signatures on all notes in a timely fashion.
- Graduate of an accredited Physical Therapy school/program.
- NYS License or eligible for licensure to practice physical therapy.
- Valid driver's license required. Reliable transportation for local travel is preferred.
