Job Title: Physical Therapist

Richmond Community Services, a leading non-profit provider of support services, is seeking a skilled Physical Therapist to join our team.

Job Summary

We are looking for a highly motivated and experienced Physical Therapist to provide support in our ICF and group home locations. The successful candidate will be responsible for planning and implementing prescribed physical therapy treatment programs for consumers to maintain and/or restore function secondary to acute or chronic disability.

Key Responsibilities
  • Conduct annual screens, assessments, and daily documentation
  • Maintain active/current MD prescriptions as outlined by the NYS Department of Health
  • Develop and implement treatment plans based on assessments
  • Administer treatment involving modalities, hydrotherapy, and exercise programs
  • Provide training to individual's families and/or direct support staff in specified follow-through programs
  • Communicate effectively with staff regarding discharge planning from the rehabilitative program
  • Participate and attend annual and semiannual conferences for all individuals receiving physical therapy services
  • Supervise the PTA (clinical supervision) and therapists on limited permits
Requirements
  • Graduate of an accredited Physical Therapy school/program
  • NYS License or eligible for licensure to practice physical therapy
  • Valid driver's license required
