Role Overview:

The RN Care Coordinator plays a vital role in enhancing patient care by offering education and support regarding social determinants of health. This position involves managing high-risk patients through comprehensive assessments, developing tailored care plans, and coordinating necessary services.

This role also includes assisting patients with various mental health conditions in accessing appropriate resources. The RN Coordinator advocates for patients and their families, ensuring they receive optimal care throughout the healthcare system.

Key Responsibilities:

  • Foster a culture aligned with our Service and Behavioral Standards.
  • Collaborate within an interdisciplinary team as part of our Patient-Centered Medical Home model.
  • Deliver nursing care following the nursing process: assessment, planning, intervention, implementation, and evaluation.
  • Develop and implement innovative programs as required.
  • Assist in the communication of clinical changes as defined by quality contracts.
  • Review inpatient utilization and discharge planning on a daily basis.
  • Provide early intervention to prevent hospitalization for high-risk patients.
  • Coordinate follow-up care with mental health services and community support.
  • Engage with patients post-discharge to ensure their needs are met.
  • Educate patients on alternatives to emergency department visits.
  • Participate in initiatives aimed at reducing emergency department utilization.
  • Facilitate changes in practice patterns with individual providers as directed.
  • Enhance care quality through ongoing education and self-assessment.
  • Support the orientation and training of new team members.
  • Collaborate with primary care providers during patient appointments.

Qualifications:

Required:

  • Graduate from an accredited nursing program.
  • Current, unencumbered Registered Nurse license in the State of Oregon.
  • Basic Life Support (BLS) certification or the ability to obtain it within 90 days of hire.
  • Minimum of 1 year of nursing experience.

Preferred:

  • Case Management certification or willingness to obtain within 2 years.
  • Demonstrated knowledge of the nursing process.
  • Strong adaptability and time management skills.
  • Excellent customer service abilities and teamwork skills.
  • Capability to manage multiple tasks in a fast-paced environment.
  • Understanding of chronic health conditions and their broader implications.
  • Self-motivated and accountable.
  • Effective communication skills in both written and verbal formats.
  • Aptitude for identifying complex problems and navigating solutions.
