Dedicated Home Health Nurse for Medically Fragile Patients

Aveanna Healthcare seeks a compassionate and skilled Registered Nurse (RN) to provide one-on-one care to adult and pediatric patients in the Acton, MA area. As a home health nurse, you will work with our patients to create personalized care plans that promote independence and overall well-being.

Responsibilities
As a Registered Nurse (RN), your primary responsibility will be to deliver high-quality, patient-centered care in the comfort of our patients' homes. Your duties may include:
- Implementing physicians' prescribed plans of care
- Conducting patient assessments and coordinating care
- Administering prescribed medication, treatments, and therapies
- Documenting care provided in our system of record
- Educating patients and family members on disease management, self-care techniques, and prevention strategies
- Collaborating with healthcare professionals to ensure comprehensive care
Requirements
To be considered for this role, you must have:
- A graduate degree from an accredited Diploma, Associate, or Baccalaureate School of Nursing
- A current, unrestricted state license as a Registered Nurse in Massachusetts
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
Salary and Benefits
We offer a competitive salary range of $54-$56 per hour, with opportunities for overtime pay.
