LPN Hospice Support Specialist

Dependable Hospice and Palliative Care is seeking a self-motivated and caring individual to join our team as a Licensed Practical Nurse (LPN) in Tucson, Arizona.
Competitive Pay: $60,000 - $80,000 per year

Responsibilities- Deliver supportive care to patients and families according to their needs and physician orders.
- Communicate with healthcare professionals to coordinate optimal care and resource utilization.
- Collaborate with nurse case managers, program directors, and medical directors to ensure seamless care delivery.
- Conduct assessments to identify patient health status and report findings to assigned nurse case managers.
- Current LPN licensure in Arizona.
- Ability to work flexible hours, including some evenings and weekends.
- Excellent communication and organizational skills.
- Valid driver's license and insurance, along with current BLS certification.
- Hospice experience a plus.
