Responsibilities

As a skilled Licensed Practical Nurse, you will be responsible for providing supportive care to patients and families in accordance with the attending physician's orders and the interdisciplinary group plan of care. This includes:

  • Identifying patient/family needs and providing supportive care
  • Working closely with the assigned Nurse Case Manager to assess and coordinate optimal care and use of resources for the patient/family
  • Maintaining regular communication with the Nurse Case Manager, Program Director, and Medical Director
  • Initiating communication with attending physicians, other hospice staff members, and other agencies as needed
Qualifications

We are seeking a self-motivated, dynamic, and caring individual to join our team as a Licensed Practical Nurse. To be considered for this role, you must:

  • Be currently licensed as an LPN/LVN in the State of Arizona
  • Have some after hours and weekend coverage experience
  • Demonstrate good verbal and written communication skills, as well as organized skills
  • Be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
  • Possess current BLS certification
  • Demonstrate skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
  • Currently possess or be able to obtain an Arizona class 1 fingerprint clearance card
  • Hospice experience is a plus
