Job Summary

We are seeking a highly skilled and compassionate Enrollment Coordinator to join our team at Brio Living Services. As a key member of our PACE program, you will play a vital role in ensuring the seamless transition of individuals into our innovative program.

Key Responsibilities
  • Educate potential members and their advocates on the PACE program and its benefits
  • Conduct face-to-face functional assessments at potential members' homes or in acute care settings
  • Identify critical care needs and collaborate with the potential member, their support system, and PACE care team to create a transitional care plan
  • Assist potential members in applying for long-term care Medicaid
  • Facilitate conversations to help individuals transition into the PACE program
  • Maintain and submit records, including enrollment paperwork, progress notes, and Medicaid documentation
  • Link individuals to and collaborate with appropriate community agencies and resources
  • Consult with and advise team members on social, emotional, and cultural factors to health and medical care, and the availability of services in the community
  • Assist with community engagement and outreach
  • Participate in quality improvement activities
Requirements
  • Active State of Michigan Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW)
  • Valid driver's license and reliable vehicle for travel (mileage will be reimbursed)
  • Minimum of one year of experience working with the frail elderly or long-term care population
