RN Care Coordination Manager

Job Description
We are seeking a talented RN Transitional Care Manager to join our team at CommuniCare Family of Companies. As a key member of our healthcare team, you will play a critical role in ensuring seamless transitions of care for our patients.
Key Responsibilities
- Discharge Planning and Coordination: Coordinate discharge planning and post-discharge care with hospital and facility teams to ensure a smooth transition for our patients.
- Care Coordination: Facilitate communication and handoff with care managers and interdisciplinary teams to ensure continuity of care and minimize complications.
- Telephonic Support: Provide telephonic support and management to case managers and nurse practitioners on complex cases, ensuring they have the necessary resources and expertise to deliver high-quality care.
- Subject Matter Expertise: Serve as a subject matter expert (SME) for case managers and nurse practitioners on transitional care best practices, providing guidance and support to ensure optimal patient outcomes.
- Documentation and Handoffs: Complete handoff forms with clinical supplementation, updated medication reconciliation, and case management recommendations, ensuring accurate and timely transfer of information.
- Complications Prevention: Identify potential issues that may lead to complications during care transitions and implement strategies to prevent them, prioritizing patient safety and well-being.
- Discharge Planning Collaboration: Collaborate with hospital discharge planners to implement discharge plans back to patient facilities, ensuring a seamless transition and minimizing delays.
- Utilization Management: Work with utilization management departments to determine appropriateness of stay and prevent unnecessary admissions, promoting efficient use of resources and optimizing patient care.
- Knowledge and Resources: Maintain knowledge of Medicare benefits, plan benefits, and community resources to provide optimal patient care and support.
Qualifications and Experience
- Licensed Registered Nurse (RN) with experience in physical and behavioral health case management, preferably in an integrated setting
- Minimum 3-5 years of case management experience, with a focus on transitional care
- Experience in Medicare Advantage programs and health coaching is a plus
- Ability to utilize various applications and databases to fulfill care management requirements and documentation
Estimated Salary Range: $85,000 - $110,000 per year, based on experience.
