As an Admissions Coordinator, you will play a critical role in facilitating the placement of patients in our dialysis centers. Your primary responsibility will be to provide exceptional customer service to patients, families, and healthcare providers, ensuring a seamless transition to our care.
Key Responsibilities- Facilitate Patient Placement: Work closely with patients, families, and healthcare providers to understand their needs and preferences, and coordinate the placement of patients in our dialysis centers.
- Provide Exceptional Customer Service: Respond promptly and professionally to inquiries, concerns, and requests from patients, families, and healthcare providers, ensuring a positive experience with DaVita.
- Collaborate with Healthcare Providers: Build and maintain strong relationships with healthcare providers, including hospitals, clinics, and physician offices, to ensure timely and efficient patient placement.
- Maintain Accurate Records: Ensure accurate and up-to-date records of patient placement, including demographic information, medical history, and treatment plans.
- Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives, including data analysis, process improvement, and best practice development, to enhance patient care and outcomes.
- Education: High school diploma or equivalent required.
- Experience: Minimum of 6 months' related experience required.
- Skills: Excellent communication and interpersonal skills, ability to work in a fast-paced environment, and proficiency in Microsoft Office applications.
