Nurse Practitioner Part-time

About the Role:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at The Laurels of Defiance. As a LPN, you will play a vital role in providing high-quality nursing care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities:
- Plan, coordinate, and provide nursing care services to residents, including health education and promotion.
- Supervise and delegate tasks to Certified Nursing Assistants (CNAs) and other team members as needed.
- Assess resident health status and notify physicians of changes or concerns.
- Develop and implement individualized care plans to meet resident needs and goals.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and resident satisfaction.
Requirements:
- Current LPN licensure in the state of employment.
- 1-3 years of experience in a long-term care setting, preferably in a skilled nursing environment.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Ability to work flexible schedules, including evenings, weekends, and holidays.
