Behavioral Health Case Manager II

Job Summary:
The Behavioral Health Case Manager II is a critical role within Elevance Health, responsible for providing comprehensive case management services to members with behavioral health and substance abuse needs. This position requires a strong understanding of clinical concepts, as well as excellent communication and interpersonal skills.
Key Responsibilities:
• Responds to complex cases and account-specific requests
• Conducts assessments to identify individual needs and develops specific care plans to address objectives and goals
• Monitors and evaluates the effectiveness of care plans and modifies them as needed
• Coordinates with internal and external resources to meet the identified needs of members and collaborates with providers
• Serves as a resource to other BH Case Managers
• Participates in cross-functional teams, projects, and initiatives
Requirements:
• Master's degree in social work, counseling, or a related behavioral health field, or a degree in nursing, with a minimum of 3 years of clinical experience in social work counseling with a broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
• Current active unrestricted license to practice as a health professional within the scope of licensure in applicable states or territory of the United States
• Previous experience in case management and telephonic and/or in-person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders
• Managed care experience required
Preferred Skills:
• Experience in health coaching and motivational interviewing techniques preferred
