Key Responsibilities- Manage and oversee clinical and administrative functions, including recordkeeping and staff supervision.
- Respond to phone inquiries and referrals in a timely and professional manner.
- Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
- Assist in the development and implementation of patient care plans, ensuring alignment with company policies and regulatory guidelines.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and patient outcomes.
- Registered Nurse with a minimum of Associate's Degree in Nursing required.
- Three years' experience in direct patient care in a clinical setting required.
- Minimum one year of management/supervisory experience required.
- Two or more years' experience in hospice or home care required.
- Current license to practice nursing in the state of employment required.
- Bachelor's Degree in Nursing preferred.
- CPR Certification required.
- Current automobile insurance and valid driver's license required.
