Key Responsibilities- Provide Patient Care: Support the care team and patients by delivering and documenting nursing care in accordance with established standards and practices.
- Coordinate Services: Assist patients in locating and coordinating services from other providers and resources.
- Document Patient Interactions: Accurately and timely document all patient interactions and clinical activities in the electronic health record (EHR).
- Triaging Patients: Triage patients according to approved protocols and consistent with ProHealth's Appointment and Scheduling Guidelines.
- Provide Education and Support: Educate patients regarding their illness or injury and provide instruction on self-management of their condition.
- Carry Out Standing Orders: Authorize prescription refills and order tests consistent with ProHealth's policies and procedures.
- Schedule Tests and Treatments: Schedule tests, treatments, and follow-up visits on behalf of patients.
- Monitor Task List: Respond to tasks in a timely manner, follow up on overdue orders, and monitor the status of critical referrals.
- Order Supplies and Vaccines: Order supplies and vaccines and maintain appropriate records.
- Prepare and Maintain Equipment: Prepare and maintain equipment, including sterilization.
- Conduct Orientation and Training: Conduct orientation of clinical support staff and provide training/coaching for staff related to clinical issues.
- Maintain Knowledge and Compliance: Maintain knowledge of current coding, OSHA, and CLIA regulations and ProHealth policies.
- Graduate of an Accredited School of Nursing: Must be a graduate of an accredited School of Nursing.
- Current LPN License: Must be currently licensed as a LPN in the State of Connecticut.
- Experience: Must have at least 1 year of LPN experience.
- Basic Computer Knowledge: Must have basic computer knowledge and aptitude.
