Job Description

As a Home Care Registered Nurse at Northwell Health, you will play a vital role in delivering high-quality patient care in a home setting. Your primary responsibility will be to develop and implement individualized care plans for patients, ensuring their safety and well-being.

Key Responsibilities:
  1. Conduct Patient Interviews and Assessments: Conduct thorough interviews with patients to understand their medical history, current health status, and personal preferences. Assess patients' physical and emotional needs, and develop a plan of care that addresses their unique requirements.
  2. Collaborate with Healthcare Team Members: Work closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
  3. Develop and Implement Care Plans: Create and implement individualized care plans that address patients' physical, emotional, and social needs. Monitor patients' progress, and make adjustments to care plans as needed.
  4. Provide Education and Support: Educate patients and their families on disease management, medication adherence, and healthy lifestyle habits. Provide emotional support and guidance to help patients cope with their conditions.
  5. Document Patient Progress: Maintain accurate and detailed records of patient interactions, assessments, and care plans. Document patient progress, and communicate with healthcare team members to ensure continuity of care.

Requirements:

  • Graduate from an accredited School of Nursing, required.
  • Bachelor's Degree in Nursing, preferred.
  • License to practice as a Registered Professional Nurse in New York State, required.
  • Valid Driver's License in New York State, required.

Salary Range: $78,000-$130,000/year
