Job Summary

We are seeking a highly skilled and compassionate Home Care Registered Nurse to join our team at Northwell Health. As a key member of our home care team, you will be responsible for providing high-quality patient care and coordination in the comfort of patients' homes.

Key Responsibilities
  • Conduct thorough patient assessments and develop individualized care plans to meet patients' unique needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Monitor patients' progress, identify potential issues, and make adjustments to care plans as needed.
  • Document patient information accurately and thoroughly, ensuring compliance with regulatory requirements.
  • Participate in quality improvement initiatives and contribute to the development of best practices in home care.
Requirements
  • Graduate of an accredited School of Nursing, with a current license to practice as a Registered Professional Nurse in New York State.
  • Specialized certifications, such as wound care or gerontology, are highly desirable.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to work independently and as part of a team, with a commitment to providing exceptional patient care and service.
