Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Care team at Northwell Health. As a Home Care Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Conduct thorough patient assessments, develop and implement individualized care plans, and provide ongoing monitoring and evaluation of patient progress.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide education and support to patients and their families, promoting health literacy and empowering them to manage their care.
  • Administer medications, perform treatments, and provide wound care as needed, adhering to established protocols and guidelines.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA standards.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and policies.
  • Stay up-to-date with the latest clinical developments, attending conferences, workshops, and other educational events to enhance your skills and knowledge.
Requirements
  • Current licensure as a Registered Nurse in New York State.
  • Graduate of an accredited nursing program, with a Bachelor's degree in Nursing preferred.
  • Valid driver's license in New York State, with a reliable vehicle for travel to patient homes.
  • Ability to work independently, with strong organizational and time management skills.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
