Job Description:

We seek a motivated Care Management Support Coordinator to join our team. As a key member of our Care Management Team, you will provide administrative support, perform outreach, and answer inbound calls to ensure seamless care management for our members. Your exceptional communication and interpersonal skills will enable you to build strong relationships with healthcare professionals and community resources.

Responsibilities:

  • Make outbound calls to existing members to discuss their care needs and enroll them in the Care Management program.
  • Support the Care Management Team by making calls, resolving issues, and documenting member records.
  • Provide data entry into True Care and maintain accurate records of member interactions.
  • Analyze and report on sales performance to meet production targets.
  • Collaborate with healthcare professionals and community resources to ensure high-quality customer care and service.

Requirements:

  • High School diploma or equivalent required; 1-2 years of experience in call centers and healthcare preferred.
  • Excellent communication and interpersonal skills, with ability to adapt to change.
  • Bilingual (Fluent in Spanish) highly desirable.

Salary and Benefits:

The estimated salary for this position is $24-$27 per hour, based on industry standards and location. BCforward offers a comprehensive benefits package, including major medical, dental, vision, group life insurance, short-term disability, long-term disability, and 401(k).
