Ambulatory Care Pharmacist

As an Ambulatory Care Pharmacist at Griffin Hospital, you will be responsible for filling and refilling patient orders during your assigned shift. You will delegate responsibilities to ensure patient care is the top priority.
You will contact and confer with physicians regarding prescriptions, reviewing and interpreting prescriptions to ensure that overdoses have not been prescribed and that incompatibilities or drug interactions will not result from combining ingredients.
You will confer with physicians to adjust drug doses based on renal and hepatic functions, patient age, and specific disease states.
You will inspect pharmacy inventory to ensure that outdated and/or potency-lacking drugs are removed from stock and replaced.
You will assume responsibility for maintaining adequate inventory by placing orders when needed.
In coordination with the primary care director, patient support leaders, director of operations, and systems team leaders, you will synthesize quality measure closure progress from internally generated quality reports.
You will serve in an educational capacity for patients and staff, providing support to Medical group staff relating to Pharmacy procedures and drug information.
You will maintain responsibility for attendance/reliability to ensure that the Medical group is operated in an efficient and cost-effective manner.
You will contribute to the mission of Griffin Hospital by supporting the Employee Philosophy, Planetree Model of Care, and organizational goals.
Education and Experience:A B.S. degree in Pharmacy is required, and State of CT registration/licensure is also required.
