Job Summary

We are seeking a highly skilled Ambulatory Care Pharmacist to join our team at Griffin Hospital. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring the safe and effective use of medications.

Key Responsibilities
  • Fill and refill patient orders during assigned shifts, prioritizing patient care and delegating responsibilities as needed.
  • Collaborate with physicians to resolve prescription-related questions and irregularities, ensuring accurate and safe medication use.
  • Review and interpret prescriptions, utilizing patient profiles to prevent overdoses and ensure medication compatibility.
  • Confer with physicians to adjust drug doses based on patient factors, such as renal and hepatic function, age, and disease state.
  • Inspect pharmacy inventory to remove outdated or potency-compromised medications, maintaining adequate stock levels.
  • Participate in quality measure closure progress, synthesizing data from internal reports to drive quality improvement.
  • Provide educational support to patients and staff, promoting pharmacy procedures and drug information.
  • Maintain attendance and reliability, ensuring efficient and cost-effective operation of the Medical group.
Requirements
  • Bachelor of Science degree in Pharmacy.
  • State of CT registration/licensure required.
