Radiology Care Coordinator

We are seeking a detail-oriented and compassionate Medical Office Nurse to join our team as a Patient Care Coordinator. In this role, you will work closely with the interdisciplinary team to develop and implement customized care plans for patients. You will also navigate patients and families across care settings according to established protocols and contribute to quality improvement initiatives.
Key Responsibilities
* Develop and implement individualized care plans for patients
* Coordinate patient care activities with the interdisciplinary team
* Communicate effectively with patients, families, and healthcare professionals
* Collaborate with the team to identify opportunities for quality improvement
* Participate in ongoing education and training to stay up-to-date with best practices in patient care
Requirements
* Two years of experience in a medical office setting
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Basic computer skills and proficiency in electronic medical records software
* Reliable transportation to travel to NYI locations for patient care needs
