Key Responsibilities- Provide individual and group therapeutic interventions to facilitate personal growth and skill building.
- Assist with assessment and personal growth planning, linking children to appropriate services, and advocating on their behalf.
- Document all services provided in accordance with identified frequency and maintain accurate records.
- Provide ongoing case management services, including assessment, planning, linking, monitoring, referral, and advocacy.
- Collaborate with healthcare providers to ensure integrated care and document physical health care in the clinical record.
- Identify and report critical incidents according to company policy.
- Participate in staff meetings, clinical staffing, and other internal meetings as assigned.
- Master's degree in social work, psychology, sociology, or other relevant behavioral science with clinical experience.
- Must be licensed or eligible for licensure.
- Excellent communication and problem-solving skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
