Care Coordinator/Life Skills Instructor

Overview:

We are seeking a dedicated Care Coordinator/Life Skills Instructor to join our team at Aspire Indiana Health, Inc. This role will play a vital part in teaching adults living with serious mental illness the skills they need to reach their individual recovery goals and connect with their community.

Key Responsibilities:

  • Utilize a strengths-based, recovery-focused approach to teach adults the skills they need to achieve their recovery goals.
  • Connect individuals with their community and provide support to help them reach their full potential.
  • Collaborate with a multidisciplinary team to provide comprehensive care and support services.

Requirements:

  • Bachelor's degree in social work, psychology, counseling, or a directly-related human service field from a college/university accredited by the US Department of Education.
  • Two years of directly related experience in professional and/or nonprofessional roles.
  • Valid driver's license, current automobile insurance, and a driving record that meets the guidelines and requirements of the organization.
