Job SummaryWe are seeking a skilled Registered Nurse Case Manager to join our Hospice team in Litchfield, Minnesota. As a key member of our team, you will provide direct care and case management for clients in accordance with agency standards and policies.
Responsibilities- Deliver skilled services efficiently and effectively to meet patients and families' needs.
- Conduct comprehensive assessments of patients and families to determine hospice needs.
- Collaborate with physicians, other healthcare professionals, clients, and families to develop a comprehensive coordinated plan of care.
- Evaluate and update the plan of care on a continuous basis, ensuring accurate communication with the multidisciplinary team.
- Promote personal safety and a safe environment for clients and coworkers.
- Graduate of an accredited school of professional nursing.
- One to two years of nursing experience in a post-acute care setting.
- Licensed as a Registered Nurse within the state in which working and maintaining licensure.
- CPR certified.
- Ability to communicate effectively verbally and in writing.
- BSN or RN with CHPN credential.
- Previous management experience.
- One to two years in Medicare Hospice experience.
