About the Role


The Transformative Behavioral Health Specialist is an integral member of our collaborative care team, reporting to Behavioral Health Leadership.


You'll work closely with patients who have primary behavioral health disorders and comorbid medical diagnoses, alongside primary care providers and licensed social workers (LCSW/LMSW) to facilitate meaningful patient engagement and person-centered care.


Key responsibilities include maintaining tailored care plans, assisting in behavioral health treatment goals, and serving as a primary liaison with community-based organizations to address social needs.


This in-person role supports patients in Syracuse, NY and surrounding counties, requiring frequent travel to patient homes and community settings.


Key Responsibilities


• Travel to patient homes and community settings to provide longitudinal care, often independently or with other team members.


• Conduct community-based outreach to locate and engage high-acuity patients, building relationships and trust.


• Meet with patients in various settings, including their homes, shelters, and community agencies, to provide care coordination and support.


• Assist patients with service coordination, including discharges from acute and alternative settings, and provide comprehensive care coordination throughout the care continuum.


• Establish strong relationships with patients, families, and the community, becoming a trusted resource and advocate.


• Preliminarily assess patients for acute behavioral and physical health needs, generating referrals to providers, community-based resources, and appropriate services.


• Document patient care and progress in a comprehensive manner, ensuring compliance with professional standards and regulatory guidelines.

  • Are flexible, able to learn quickly, and adaptable to changing environments.
  • Hold a Bachelor's degree in a related field, with 2+ years of experience providing care coordination/case management, or a Master's degree in a related field and 1 year of experience.

Compensation


$28-32/hour, based on prior experience, performance, and market dynamics.


Physical Requirements


Employee must be able to meet the following requirements with or without reasonable accommodation: the ability to stand, sit, and walk for extended periods of time, and the ability to lift and carry objects weighing up to 20 pounds.


The job involves frequent bending, reaching, and kneeling to perform various tasks.


Travel Requirements


This role involves frequent travel to patient homes and community settings, with approximately 60% of time spent in the field and the rest working remotely.
