As a Case Manager II, you will play a vital role in our community outreach program, serving 60 adults with serious, persistent mental illness, and/or dual diagnosis.
Key Responsibilities:
- Provide primary support and case management for individuals with mental illness and co-occurring disorders.
- Assist clients to live as independently as possible in their chosen community.
- Under direct supervision, serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders.
What You Bring to the Table:
- High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services.
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
