About the Role

We are seeking a skilled and compassionate Case Manager II to join our team at Telecare Sonoma ACT. As a primary support and case manager, you will work directly with individuals with mental illness and other co-occurring disorders to help them live as independently as possible in their chosen community.

Key Responsibilities
  • Provide direct support and case management services to individuals with mental illness and other co-occurring disorders
  • Assist clients in developing and implementing individualized treatment plans
  • Coordinate with healthcare providers, family members, and other stakeholders to ensure comprehensive care
  • Provide transportation and accompaniment to appointments and groups
  • Document client progress and maintain accurate records

Requirements

  • Willingness to use your personal vehicle to drive clients to appointments and groups
