Key Responsibilities- Provide direct support and case management services to individuals with mental illness and other co-occurring disorders
- Assist clients in developing and implementing individualized treatment plans
- Coordinate with healthcare providers, family members, and other stakeholders to ensure comprehensive care
- Provide transportation and accompaniment to appointments and groups
- Document client progress and maintain accurate records
- A competitive salary range of $21 - $23.71, with geographic differentials applied
- A sign-on bonus of $1,000
