Job Description

Caminar is seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will provide support and assistance to psychiatrically disabled individuals in the community college setting, promoting their educational goals and well-being.

Key Responsibilities
  • Develop and implement individualized plans to support clients' educational and rehabilitation goals.
  • Provide linkage and brokerage services to additional resources and supports as needed.
  • Collaborate with DSPS staff, Career Center, Learning Disabilities Center, and other agencies to ensure seamless support services.
  • Teach skills needed to succeed in campus, such as time management, symptom management, and other skills.
  • Document treatment planning and interventions according to agency, county, and MediCal billing and quality assurance requirements.
  • Drive own or agency vehicle to educational and treatment destinations, as required.
Requirements and Qualifications
  • High School Diploma or GED required; Bachelor's degree in psychology or mental health related field preferred.
  • Minimum one year of demonstrated work experience with SMI/DD preferred.
  • Excellent computer skills in a Microsoft Windows environment, including email and Internet navigation.
  • Ability to maintain a high level of confidentiality and represent the organization in a positive manner at all times.
