Job Title: Case Manager, Supported Education

Caminar is seeking a highly motivated and compassionate Case Manager to join our Supported Education team. As a Case Manager, you will provide support and assistance to psychiatrically disabled individuals in the community college setting, helping them achieve their educational goals.

Key Responsibilities:
  • Develop and implement individualized support plans to help clients achieve their educational goals.
  • Provide linkage and brokerage services to additional services and supports as needed for the attainment of other rehabilitation goals.
  • Collaborate with the Disabled Students Programs and Services (DSPS) staff, the Career Center, Learning Disabilities Center, Psychological Services, and other agencies within the campus to ensure seamless support for clients.
  • Present and promote the services of the Supported Education program to other Caminar programs and community providers/members.
  • Assist clients in developing skills needed to succeed in campus, such as time management, symptom management, and other skills, as necessary.
Requirements:
  • High School Diploma or GED is required. Bachelor's degree in psychology or mental health related field is preferred.
  • Minimum one year of demonstrated work experience with SMI/DD is preferred.
  • Excellent computer skills in a Microsoft Windows environment, including email and Internet navigation, are required.
  • Ability to maintain a high level of confidentiality, a professional demeanor, and to represent the organization in a positive manner at all times.
