Job Summary

This Medical Care Coordinator will work closely with our care team to provide comprehensive, patient-centered care in the patient's home. With a focus on delivering timely care that reduces avoidable hospitalizations and maximizes quality of life, you will play a critical role in improving patient health and satisfaction.

Responsibilities
  • Conduct comprehensive annual wellness exams and assess patient health status
  • Counsel and educate patients and families about benefits and programs available to support healthier living
  • Document patient information, including chief complaint, diagnosis, past medical history, review of systems, examinations, medications, allergies, assessment, and plan
  • Coordinate care with primary care providers, specialists, and ancillary services as needed
Requirements

To be successful in this role, you must possess:

  • A strong background in direct patient care, with 2+ years of experience preferred
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Bilingual (English/Spanish) highly preferred
  • Basic Life Support (BLS) certification required
Salary and Benefits
