This challenging yet rewarding role involves coordinating hospital case management services to ensure efficient, cost-effective patient progression from admission through discharge. We are seeking a skilled professional with an Associate's Degree in Nursing and at least 5 years of experience in an Acute Care setting. Discharge Planning expertise is highly valued but not essential. Responsibilities:
- Develop and implement comprehensive care plans to meet the unique needs of each patient
