Job Summary:

Mozaic Senior Life is seeking a skilled and experienced MDS Coordinator to join our team. As a key member of our healthcare team, you will be responsible for ensuring the highest quality of care for our residents.

Key Responsibilities:

  • Conduct thorough assessments of residents' medical conditions and develop individualized care plans.
  • Collaborate with nurse managers and interdisciplinary teams to ensure that care plans reflect residents' changing needs.
  • Ensure that all Medicare residents receive skilled care and services.
  • Manage and coordinate the MDS process, including initial admissions, quarterly updates, and changes in condition.

Requirements:

  • Graduate of a school of professional nursing; BSN preferred.
  • Strong clinical background in nursing, with experience in care planning and MDS.
  • Fluency in English and ability to communicate effectively with residents, families, and healthcare teams.
  • RN licensure in the State of Connecticut and CPR certification/IV-line management 3.0 certification.
