Overview
We are seeking a highly organized and knowledgeable Patient Care Coordinator to join our team at AccentCare. As a Client Service Coordinator, you will play a vital role in providing outstanding patient care and ensuring the quality coordination of services for our clients and Care Partners.
In this role, you will be responsible for monitoring and supporting the daily activities of services being provided to clients in accordance with state regulations and company policy. You will also function as primary support for daily EVV and payroll functions, process and communicate authorization and schedule changes, and coordinate utilization of Care Partner teams to ensure fill rate expectations are met.
The ideal candidate will have previous home health care or home care experience, a high school diploma or GED, and excellent communication skills. A valid driver's license is required.
Salary: $53,000 per year (based on 26.00 per hour * 40 hours/week * 52 weeks/year)
