**Job Title:** Patient Advocacy Coordinator

We are seeking a dedicated and compassionate Patient Advocacy Coordinator to join our team at Alameda Care Hospice. In this role, you will serve as a liaison between patients, families, and healthcare providers, ensuring that patient needs are met and advocating for their rights and interests.

**Annual Salary Range:** $70,000 - $85,000

**Key Responsibilities:

  • Represent patient interests and concerns to healthcare providers and other stakeholders
  • Facilitate open communication between patients, families, and healthcare teams
  • Identify and resolve patient complaints and issues

**Requirements:**

  • Master's degree in Social Work or related field
  • Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
  • At least 2 years of experience in patient advocacy or related field
