Job Overview:

As a Hospice Care Specialist, you will be responsible for providing high-quality care to patients and their families in the comfort of their own homes. Your primary focus will be on conducting thorough assessments, developing personalized care plans, and coordinating with healthcare teams to ensure seamless delivery of services.

Key Responsibilities:

  • Patient Care: Conduct comprehensive assessments, follow established care plans, and provide emotional support to patients and families.
  • Medication Management: Administer medications accurately and document treatments thoroughly.
  • Care Coordination: Communicate effectively with healthcare teams and families to address patient needs and preferences.
  • Patient Education: Provide clear guidance on care plans, medication use, and disease management.
  • Documentation: Maintain accurate and up-to-date patient records.

Requirements:

  • Education: Diploma, Associate, or Bachelor's degree in Nursing (BSN preferred).
  • Experience: 1+ year in adult, pediatric, or medical/surgical clinical care (hospice experience preferred).
  • Licensure: Licensed to practice as a Registered Nurse in New Jersey; PA licensure if practicing in PA.
  • Skills: Strong assessment, documentation, and communication skills; proficiency in Microsoft Office and electronic medical records.
