Job Description
As a Hospice Case Manager, you will play a vital role in coordinating and delivering comprehensive palliative care services to patients and their loved ones. Your primary responsibilities will include assessing patient needs, developing personalized care plans, and collaborating with interdisciplinary teams to ensure seamless care transitions.
Key Responsibilities:
- Conduct thorough physical and psychosocial assessments of patients to identify their unique care requirements
- Develop and implement individualized care plans that address patients' medical, emotional, and social needs
- Collaborate with hospice staff, physicians, and other healthcare professionals to ensure comprehensive and coordinated care
- Provide education and support to patients and their families to empower them in managing their care
- Monitor patient progress, adjust care plans as needed, and document all interactions and interventions
Requirements
To be successful in this role, you will need:
- A Diploma, Associate, or Bachelor of Science in Nursing (BSN) degree from an accredited institution
- Licensure as a Registered Nurse in the State of New Jersey (NJ) and licensure in Pennsylvania if practicing in PA
- Minimum one-year adult, pediatric, or medical/surgical clinical experience
- Strong nursing assessment and documentation skills, clinical knowledge, and ability to communicate effectively with patients and families
