Key Responsibilities- Provide Relationship-Based Patient Centered Care
- Assess patient needs and develop individualized care plans
- Coordinate care through effective partnerships with patients, families, and healthcare teams
- Adapt service delivery to reflect an understanding of cultural diversity
- Practice Evidence-Based Care
- Perform the nursing process (assessment, diagnosis, identification of outcomes, planning, implementation, and evaluation) in the performance of clinical care
- Use analytical and critical thinking to ensure that care is individualized and coordinated
- Collaborate with Interdisciplinary Teams
- Coordinate the delivery and documentation of safe, quality patient care
- Participate in interdisciplinary rounds, case review, and other communication methods
- Participate in Performance Improvement
- Participate in performance improvement activities, including research and research utilization
- Initiate and/or lead performance improvement activities to promote safe, quality patient care
- Develop and Maintain Professional Competencies
- Complete initial and ongoing professional competency assessments
- Participate in mandatory education and population-specific education
- Other Responsibilities
- Perform other duties as assigned
- Comply with all Wellstar Health System policies, standards of work, and code of conduct
- Education
- Associate's Degree in Nursing or Graduate of accredited/approved school of nursing
- Bachelor's Degree in Nursing preferred
- Experience
- Minimum 2 years direct patient care nursing experience
- Licenses and Certifications
- Registered Nurse (Single State) required
- RN - Multi-state Compact required
- Basic Life Support required
- BLS - Instructor required
- BLS - Provisional required
