Job Title: Registered Nurse (RN) Care Manager

LHC Group is seeking a skilled Registered Nurse (RN) to fill a Care Manager position in our Home Health division. As a Care Manager, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Job Summary:

The successful candidate will be responsible for supervising and coordinating clinical services, ensuring that patients receive the best possible care. This includes evaluating patient needs, developing and implementing care plans, and collaborating with other healthcare professionals to achieve optimal patient outcomes.

Responsibilities:
  • Provide clinical services within the scope of practice, as defined by state laws governing nursing practice.
  • Supervise and coordinate clinical services, ensuring that patients receive high-quality care.
  • Evaluate patient needs and develop and implement care plans that meet individualized patient needs.
  • Collaborate with other healthcare professionals to achieve optimal patient outcomes.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
