Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at UMass Memorial Health Care. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities- Assess and care for patients in all settings, including obtaining patient histories, administering medications, and monitoring vital signs.
- Rend and supervise care of clients on the unit, ensuring the highest level of patient care.
- Monitor medication and consult with prescribers to ensure accurate and safe medication administration.
- Provide education and adherence to prescribed treatment protocols, promoting patient wellness and recovery.
- Facilitate wellness and medically related psychoeducational groups to individuals, promoting patient empowerment and self-care.
- Record all client information within the electronic medical record system daily, ensuring accurate and up-to-date patient records.
- Assist RNs on a daily basis, providing support and contributing to the success of the healthcare team.
- Offer nursing consult on emergency service evaluations, providing expert guidance and support.
- Work with clients to coordinate care with their PCP, psychiatrist, and other professionals, ensuring seamless patient care.
- Demonstrate ability to communicate effectively and professionally with clients, staff, and providers, promoting a positive and respectful work environment.
- Demonstrate proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
- Knowledge of medical, substance abuse, and mental health nursing, with a focus on providing high-quality patient care.
- Respect diverse views and approaches, demonstrating Standards of Respect, and contributing to creating and maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.
- Grade N04: Acute Care LPN: Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
- Previous clinical experience working with substance abuse and mental health.
- Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives.
- All bilingual candidates are strongly encouraged to apply.
- Current valid US-issued driver's license and ability to provide registered, inspected, and insured automobile for work-related purposes, including transporting clients in your own vehicle.
- All candidates will be subject to a CORI review as outlined in the regulations set by the Massachusetts Executive Office of Health and Human Services.
