Job Summary

We are seeking a skilled Licensed Practical Nurse to join our Home Health team. As a LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to achieve exceptional patient outcomes.

Key Responsibilities
  • Provide patient care according to established standards and policies.
  • Assist with patient education and document patient information accurately.
  • Collaborate with healthcare professionals to ensure continuity of care.
  • Administer medications safely and in accordance with professional standards.
  • Make recommendations for patient care to RNs and other team members as needed.
  • Delegate patient care activities to appropriate personnel.
  • Meet productivity guidelines and support performance improvement initiatives.
  • Maintain accurate and complete documentation.
  • Ensure a safe environment for patients through consistent compliance with policies and procedures.
Requirements
  • Current licensure as a Licensed Practical Nurse in Massachusetts.
  • Successful completion of an accredited LPN program.
  • Valid US driver's license and registered, inspected, and insured automobile for work-related purposes.
Preferred Qualifications
  • BLS certification.
  • CPR certification.
