Job Summary

We are seeking a highly skilled and motivated Licensed Practical Nurse (LPN) to join our team. As an LPN, you will work closely with our experienced healthcare professionals to provide high-quality patient care in a variety of settings, including post-acute facilities. Your expertise and compassion will be invaluable in helping us deliver exceptional patient outcomes and exceed our patients' expectations.

Responsibilities
  • Provide direct patient care, including administering medications, monitoring vital signs, and documenting patient information.
  • Collaborate with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and mental health, identifying potential risks and developing strategies to mitigate them.
  • Administer medications and treatments as prescribed by physicians, ensuring accurate dosing and monitoring for potential side effects.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Current Massachusetts state license to practice as a Licensed Practical Nurse (LPN).
  • Completion of an approved Licensed Practical Nurse (LPN) educational program.
  • High school diploma or equivalent required.
  • Ability to lift a minimum of 50 pounds and stand for long periods.
  • Minimum of 6 months of experience in a healthcare setting, preferably in a post-acute care environment.
