Community-Based Mental Health Counselor / QMHP
EMS of Virginia is in search of a Qualified Mental Health Professional (QMHP) who is registered with the Board of Counseling to deliver Intensive In-home and Mental Health Skill Building Services. This role will primarily serve clients in various communities, emphasizing personalized care and support.
Since our establishment in 2005, EMS of Virginia has dedicated itself to addressing the mental health requirements of the Tri-Cities area. Key Responsibilities:
- Conducting client sessions in community settings, including their residences
- Providing individualized mental health counseling aligned with client objectives
- Facilitating skill development for adults experiencing serious mental health challenges
- Accompanying adult clients to various appointments
- Connecting adult clients with community resources
- Managing cases to ensure clients' fundamental needs are fulfilled
- Offering individual and family counseling to youth
- Developing personalized service plans
- Preparing quarterly evaluations
- Documenting session progress notes for each client interaction
- Assisting with service authorization documentation
- Participating in regular supervision meetings
- Attending team collaboration sessions
