Job Summary

We are seeking a highly skilled and compassionate Licensed Vocational Nurse (LVN) to join our Patient Support Center team at Texas Health Action. As a Patient Support Center LVN, you will play a critical role in providing exceptional patient care and support to our clients.

Key Responsibilities
  • Interpret lab results, send medication refills, and schedule treatments in accordance with our organization's clinical standards and procedures.
  • Follow Texas Department of State Health Services (DSHS) notifiable condition reporting requirements.
  • Provide patient education on disease processes, therapies, and healthful behaviors.
  • Ensure accurate and timely documentation of patient consultations and treatments in their medical records.
  • Prioritize work tasks and services to ensure efficient and effective patient care.
  • Maintain accurate, clear, and concise progress notes, problem lists, and medication lists.
  • Communicate effectively and tactfully with staff, consultants, patients, partners, and community stakeholders.
  • Reduce conflict and increase patient satisfaction through timely and empathetic responses.
  • Participate in staff meetings, planning sessions, and in-service training as needed.
  • Assist with medication prior authorizations and other duties as assigned.
Requirements
  • Current LVN licensure in the state of Texas.
  • At least 2 years of experience in patient care, preferably in an outpatient clinic setting.
Preferred Qualifications
  • Telephony patient care experience.
  • Bilingual English/Spanish language proficiency.
  • Experience working with diverse populations, including LGBTQ+ and other marginalized communities.
