Job Description

We are seeking a highly skilled Care Coordinator to join our team at Integrated Services for Behavioral Health. As a Care Coordinator, you will play a critical role in providing exceptional care to children, youth, and families with significant behavioral health needs.

Key Responsibilities:

  • Develop and implement comprehensive care plans that address the unique needs of each child, youth, and family.
  • Collaborate with community agencies and service providers to ensure seamless care coordination and service delivery.
  • Engage and build positive relationships with children, youth, and families to support successful integration of care plans.
  • Maintain accurate and up-to-date documentation of care plans, progress, and outcomes.
  • Participate in program evaluation and quality improvement initiatives to ensure high-quality care delivery.

Requirements:

  • Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field.
  • Three years of experience with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher.
  • Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
  • Two years of experience in a coordinated supportive services or care coordination role is preferred.
  • Experience working with people with autism spectrum disorders and developmental disabilities is preferred.
