Key Responsibilities:
- Provide hands-on post-acute care planning services to patients, as assigned by an RN Case Manager or Social Worker.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare professionals to ensure smooth transitions.
Requirements:
- Graduate from an accredited school of nursing.
- Current licensure as a practical nurse in Nebraska or a compact state.
- Basic Life Support (BLS) certification from an American Heart Association (AHA) endorsed program.
