Key Responsibilities:

  • Provide hands-on post-acute care planning services to patients, as assigned by an RN Case Manager or Social Worker.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure smooth transitions.

Requirements:

  • Graduate from an accredited school of nursing.
  • Current licensure as a practical nurse in Nebraska or a compact state.
  • Basic Life Support (BLS) certification from an American Heart Association (AHA) endorsed program.
