Key Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Conduct thorough assessments, develop care plans, and implement interventions to meet individual resident needs
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective communication
  • Document resident interactions, outcomes, and care provided in a timely and accurate manner

Requirements:

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
  • Ability to work effectively in a fast-paced environment, prioritize tasks, and maintain a high level of productivity
