**Responsibilities**

As an LPN Health Care Specialist, you'll play a vital role in supporting our healthcare teams by ensuring they have everything they need to care for our patients. Your responsibilities will include:

  • Performing general nursing care to patients by following established clinic workflows
  • Obtaining patient consent for care and ensuring patient confidentiality
  • Obtaining and documenting health history, information on chief complaint, vital signs, and health risk factors, and assessing disease conditions
  • Providing patient care based on practice guidelines, standards of care, and federal/state laws and regulations
  • Executing prescribed treatments and medical interventions, administering prescribed medications, and monitoring and documenting treatment progress and patient response
  • Educating patients and families about health status, health maintenance, and management of acute and chronic conditions
  • Documenting patient assessment and intervention data using established medical record forms/automated systems and documentation practices
  • Participating in multidisciplinary teams to improve patient care processes and outcomes

**Qualifications**

To be considered for this role, you'll need:

  • A graduate degree from an accredited Licensed Practical Nursing program
  • A current LPN license in good standing with the state licensing board
  • Basic Life Support (BLS) certification from the American Heart Association
  • Completion of an IV therapy course

**Estimated Salary:** $65,000 - $85,000 per year, depending on experience and qualifications.
